Scenario 1
The patient receives only PT 3 times per week. The SOC is April 1, 2014. Visit 13 occurs on April 29, 2014 and a reassessment is required. This reassessment has occurred within the first 30 days so an additional reassessment is not required until visit 19 assuming continued PT is still medically necessary.
