Scenario 2
The patient received only PT twice a week with the SOC of May 1, 2014. The 30 day Reassessment is required by May 31, 2014. This would be visit 10. An additional Reassessment would be required at visit 13.
The Reassessment on May 31, 2014 could be performed at an earlier visit if desired, but must be completed no later than May 31, 2014.
