PT Only - Visits twice a week with a SOC of May 1, 2014. A Reassessment visit is not performed until visit 13 on June 10, 2014, day 40. The Reassessment required every 30 days was missed. The therapy visits provided by a PTA on May 31, 2014 - June 4, 2014 - June 7, 2014 are not covered and made non-billable. The visit at which the Reassessment was performed (June 10, 2014) would be covered.
The non-covered visits are not counted when determining when the next reassessment is needed. This means that while the patient has received 13 PT treatments, for counting purposes, the patient has only received 10 visits. Assuming the patient continues to need PT, another Reassessment will be required after an additional 3 visits.
