Scenario 4
A patient needs PT 2 days per week with a SOC on May 1, 2014. OT is also needed 2 days per week with a SOC of May 6, 2014. Visit 13 could occur on May 23, 2014.
The PT Reassessment is completed at visit 13. The OT Reassessment is not completed until visit 14.
Visit 14 will be covered, but the OT visit closet to visit 13 (either visit 11 or 12) will not be covered if it was a COTA. If it was an OT, add a call log if able. If not, this would also be non-billable.
For counting purposes, visit 14 becomes visit 13.
