~ Heart Failure Educational ~

As part of our American Heart Association–Certified Heart Failure Program, Tufts Medicine Care at Home (TMCAH) provides evidence-based Heart Failure education modules for all staff who care for patients living with heart failure. These education modules are designed to strengthen early recognition of heart failure changes, improve patient self-management, and support our ongoing efforts to reduce preventable hospitalizations. Each module focuses on a key clinical or patient-care topic and includes resources tailored to multiple disciplines.

Clinicians are required to complete one (1) hour of Heart Failure education annually. New hires are provided with a PowerPoint detailing Heart Failure Education and Module One (1) as part of their onboarding experience.

Listed below are evidence-based, best practices, Heart Failure Modules that are designed to reach the goals of: A. to strengthen early recognition of heart failure changes, B. improve patient self-management, and C. support our ongoing efforts to reduce preventable hospitalizations. Module links are listed below accompanied with related resources and required assessments. Upon Completion of the two modules you take, your completion of this education will be tracked for clinical managers have access to. Please be sure to take the attestation or and/or a quiz if part of module. 

 Each module is counted as 30 minutes of education time. therefore, you should complete two (2) modules of the various modules available.

Module 1:

This 3-Part Series can be completed together or separately for annual learning and will be assigned in its entirety for new hires. Regardless of how/when it is taken, the attestation must be complteted after all three parts Module 1 are completed.
