Home Health Therapy Reassessment 2013
When a missed Reassessment is identified:
1. The therapist made a visit on the required date(s), but without a Reassessment:
- The CSC will contact the therapist and request the therapist to enter a
call log if enough objective measurements were done in order to qualify as
as a Reassessment. If unable to add call log, notify Patty Chace or
Laurie Allore.
2. A therapy assistant made a visit on the required date(s) of the Reassessment:
- QA will make the therapy assistant visit non-billable and the visit will not be
included in the therapy count.
3. The therapist did not make a visit on the required date(s):
- The CSC will tag next visit or, if already made, will notify the therapist
who made the next visit to add a call log if able.
