Home Health Therapy Reassessment 2013
Home Health VNA:
1. CSC (Patty Chace/Laurie Allore) will run a daily report and tag appointments where a reassessment visit is needed. Patty Chace will be reviewing all sites except Seacoast. Laurie Allore will be reviewing the Seacoast site.
2. Identify missed reassessments within the past week.
3. Since the information in these reports varies throughout the day (due to changes in clinician schedules, patients cancelling visits, MD appointments, etc), the CSC’s may need to contact the clinicians to alert them if a reassessment is due or has recently been missed.
4. If you need to make changes in your schedule during the day, that may affect a Reassessment for another clinician. Please call your CSC ASAP so that they can contact the appropriate/affected clinicians.
5. Please synchronize your computers during the day to capture any changes made in your schedule.
6. Please do not disregard the tags in your schedules.
7. It will be very important for clinicians to communicate with each other, especially when planning to discharge a patient receiving other therapies. The more advance notice each clinician has, the better.
