Home Health Therapy Reassessment 2013
The Reassessment documentation must
include:
Functionally reassess the patient using a method that objectively measures activities/functions of daily living, compare the resultant measurement to the prior assessment measurements, and document in the clinical record the measurement results along with your determination of the effectiveness of therapy or lack thereof.
Review the goals on the POC for the following:
1. The goals have been reviewed
2. The goals have been achieved and remain appropriate
3. The goals require updating
4. The patient continues to make progress towards the goals (improvement
is evidenced by objective successive measurements)
5. Include any problems the patient is having, affecting their ability to reach the
goals
Justification for continuation of skilled PT/OT/ST services.
Any discharge plans if appropriate.
