Therapy Reassessment:
Single Discipline
1. Reassessment must be on Visit 13 & Visit 19. Count restarts with new
episode.
Multi Discipline
1. The 13th Reassessment to be completed by the 13th Visit, but no earlier than
the 11th Visit.
2. The 19th Reassessment to be completed by the 19th Visit, but no earlier than
the 17th Visit.
3. Count restarts with new episode.
30 Day Reassessment
1. All disciplines: Each discipline reassesses at least every 30 days.
2. Any reassessment visit resets the 30 Day clock for that discipline. Count
continues across episodes.
