Therapy Assessment & Reassessment:


Assess the patient using a method that allows for objective measurement of function and successive comparison of measurements. For each therapy discipline for which services are provided, the qualified therapist (instead of an assistant) must assess the patient's function using a method that objectively measures activities of daily living:
    1. ROM measurements
    2.  Skilled gait assessment
    3.  Ability to perform ADL
    4.  Assessment of pain
    5.  Strength measurements
    6.  Swallowing assessment
    7.  Speech assessment

If more than one discipline of therapy is provided, the qualified therapist from each of the disciplines must functionally assess the patient and comprehensively document the measurement results that correspond to his or her discipline and care plan goals in the clinical record.
